Title: CARE-link: A Web-Based Electronic Health Record System for Enhancing Diabetes Management Through Remote Engagement

Abstract: CARE-link is an open-source, web-based clinical support platform aimed at improving the management of gestational diabetes. It connects clinicians and patients via a workflow mediated by a Large Language Model (LLM). The system collects patient-generated data from outside hospital settings, synthesizes pertinent clinical details, and provides context-aware decision support for medical professionals. On the patient side, CARE-link offers clear explanations of treatment plans and sends timely lifestyle advice through WhatsApp. This dual-facing design encourages ongoing monitoring, supports personalized care, and decreases the need for in-clinic follow-ups. With its modular architecture, the platform can be adapted for other chronic conditions that require long-term tracking and behavioral support. CARE-link holds the potential to improve clinical oversight, boost patient adherence, and enhance care continuity, especially in resource-limited environments.
